Following the announcement by the JCVI on 29th November, that all adults over the age of 18 will be offered a Covid Booster jab 3 months after their second dose, we are waiting for more operational details so that we can plan how to vaccinate the local community as quickly and safely as possible. More details will be added to this website as soon as we have them. Please DO NOT phone the surgery to make enquiries, we will contact you when it is your turn. Thank you.

Integrated Care Team

A group of local healthcare professionals, working together as a team to help patients who are referred to them (with their consent) because they have complex needs with which more than one organisation may help.

The team is led by Dr Gareth Robinson, GP and includes a Social Worker, a Mental Health Practitioner, Therapists, Specialist Nurses, Talk Plus (counselling), Making Connections (social prescribing).

Integrated Care Team Co-ordinator: Zeph Storr

Clinical Pharmacist:  Dilvir Gorae

Paramedic Practitioners:  Frank O'Connell, John Liddell and Sharon Coleman

Practice Community Matron: Melissa Leach

Mental Health Practitioner: Sharon Burns

Referrals to the team are usually made by healthcare professionals such as the Community Nurses or the Ambulance Service or by the Doctors, but families or friends who are concerned can speak to Zeph Storr, the Integrated Care Team Co-ordinator for the group, who is based at Yateley Medical Centre for advice about whether a referral to the group would be helpful.

The feedback from patients and their families who have been helped by the team is very positive indeed.
